Throughout all of our services we work closely with statutory, private and voluntary sector partners to ensure customers are given the support they need to move on from the cycle of homelessness.
The Homeless Street Triage (HOST) car
The HOST car introduced on a trial basis in August 2015 – responds to the growing number of calls reporting homelessness, begging and anti-social drinking in Birmingham city centre.
Working in partnership with our partners Birmingham City Council and West Midlands Police we are able to provide a triage service to engage rough sleepers and direct them to benefits support, housing, alcohol and substance misuse services, or mental health support.
Payment by Outcomes
Midland Heart has been responsible for successfully delivering one of England’s largest payment by outcomes contracts for vulnerable people in partnership with Birmingham City Council.
Payment by Outcomes, also known as Payment by Results (PbR), is central to Government’s plans for changing the way public services are delivered. The concept supports service providers to replace centrally imposed targets with a system of payment by results that offer financial awards to agencies delivering services, while also helping to evidence what is working in commissioned services for vulnerable people.
Our Birmingham based supporting people services have consistently achieved high outcomes and lead the way in this innovative form of contract delivery.
